Description
Running along the medial (ulnar) side of the male upper limb, the basilic vein is traced from the superficial venous network of the hand and forearm toward the cubital fossa, where it lies medial to the biceps brachii tendon and continues proximally along the medial arm. A communicating channel at the elbow, consistent with the median cubital vein, bridges toward the more lateral cephalic vein across the anterior aspect of the elbow. Deep to the superficial fascia, the brachial artery courses in the anterior compartment of the arm, with the biceps brachii anteriorly and triceps brachii posteriorly framing the vessel as it descends toward the elbow. Distally, metacarpals and phalanges anchor the orientation of the wrist and hand. Medial emphasis matters because the basilic vein transitions from a superficial conduit to a deeper vessel as it pierces the brachial fascia in the mid to proximal arm to join the brachial veins and form the axillary vein. That anatomic shift is a procedural trap. Peripheral cannulation is often easier at the cubital fossa via the median cubital connection, while the proximal basilic course is commonly targeted for peripherally inserted central catheter placement and upper arm venous access, where proximity to the medial antebrachial cutaneous nerve can explain post-procedural dysesthesia.
