Description
Talus body anatomy dominates this posterior view, with the posterior process forming the inferior margin and the posterior articular surface facing toward the calcaneus. Medially and laterally, the posterior process separates into the medial and lateral tubercles, creating a central sulcus that continues as the groove for the tendon of flexor hallucis longus. Superior to the posterior process, the trochlear surface of the talar body curves toward the ankle mortise, while the more inferior contour blends into the posterior facet that participates in the subtalar joint. Bony relief is crisp. Posterior talar morphology matters whenever heel pain localizes to the back of the ankle or when plantarflexion provokes symptoms. Hypertrophy or an unfused accessory ossification center at the lateral tubercle (os trigonum) can narrow the posterior ankle recess and irritate the flexor hallucis longus within its groove, a common mechanism in posterior ankle impingement seen in ballet dancers and soccer players.
