Description
Cerebellopontine angle (CPA) anatomy is centered on the lateral aspect of the pons where it meets the anterior surface of the cerebellar hemisphere. From this fixed lateral perspective, the pontine bulge lies anteromedially while the cerebellar flocculus and adjacent cerebellar hemisphere sit posterolaterally, framing the CPA cistern between them. Cranial nerve VIII (vestibulocochlear nerve) is highlighted as it courses laterally toward the internal acoustic meatus, closely accompanied by cranial nerve VII (facial nerve) in the same corridor, with the middle cerebellar peduncle forming a prominent posterior boundary and the inferior cerebellar peduncle appearing more caudally. Arterial neighbors such as the anterior inferior cerebellar artery (AICA) typically loop near this nerve complex, and the relationship to the petrous part of the temporal bone is implied at the far lateral margin. CPA relationships matter because this small interval is where common extra-axial lesions declare themselves first. Vestibular schwannoma and meningioma classically expand within the CPA cistern, displacing CN VII and CN VIII and compressing the lateral pons, correlating with unilateral sensorineural hearing loss, tinnitus, vertigo, and later facial weakness or numbness from trigeminal involvement. The lateral view helps you teach the surgical corridor used in retrosigmoid and translabyrinthine approaches, where the facial and vestibulocochlear nerves must be differentiated and protected as they converge toward the internal acoustic canal.
