Description
Arising inferior to the larynx, the cervical trachea descends in the midline with its C shaped cartilaginous rings visible through to the thoracic inlet, then divides at the carina into the right and left main bronchi. From an anterior perspective, the right principal bronchus tracks slightly more vertical and lateral than the left, beginning the bronchial tree with early lobar branching toward the right upper lobe and the left upper lobe. Proximally, the airway calibre is broad and regular, then narrows distally as segmental bronchi begin to diverge within each lung field. Bifurcation anatomy is the focus. This anterior tracheobronchial view matters because small differences in bronchial angulation drive real clinical decisions. The right main bronchus is shorter, wider, and more in line with the trachea, so aspirated material and misplaced endotracheal tubes commonly enter the right lung, producing right upper lobe or right lower lobe atelectasis depending on the level of obstruction. Carinal widening or blunting is a classic teaching landmark for subcarinal lymphadenopathy and left atrial enlargement, and it also guides bronchoscopy orientation when sampling mediastinal nodes or evaluating hemoptysis. Use it to support gross anatomy and respiratory physiology teaching on conducting airways, mucociliary clearance, and airflow distribution, and to anchor clinical modules on aspiration pneumonia, foreign body airway obstruction, and bronchoscopy navigation. It also fits radiology and pulmonology publications that correlate airway branching with chest radiograph and CT findings, including mainstem intubation and post obstructive collapse.
