Description

Positioned inferior to the occipital lobes, the cerebellum fills the posterior cranial fossa with two cerebellar hemispheres flanking the midline vermis. Deep transverse fissures and tightly packed folia define the posterior surface, with lobular contours suggesting the posterior lobe and the paramedian portion of the vermis. At the superolateral margins, the cerebellar cortex sweeps toward the cerebellar peduncles, the fiber bundles that connect the metencephalon to the pons and midbrain and sit anterolateral to the cerebellar mass. Posterior orientation makes cerebellar symmetry easy to teach and, more importantly, frames how clinicians think about midline versus hemispheric signs. Midline vermian injury commonly presents with truncal ataxia and gait instability, while hemispheric involvement tends to produce ipsilateral limb dysmetria and intention tremor, a pattern used daily when localizing infarct, hemorrhage, tumor, or demyelinating plaque in the posterior circulation territory. It is a clean way to tie surface anatomy to bedside tests such as finger to nose, heel to shin, and rapid alternating movements. Motor control lives here.
