Claustrum appears as a thin, vertically oriented lamina of gray matter on the medial aspect of the cerebral hemisphere, positioned lateral to the putamen and medial to the insular cortex. Medially, the external capsule forms a narrow white matter plane separating the claustrum from the lentiform nucleus, while laterally the extreme capsule intervenes between claustrum and insula. The sheetlike geometry is emphasized by its superior to inferior extent, paralleling the curvature of the insular region and bordering the basal ganglia complex in close proximity. Edges are crisp. Anatomists and clinicians often struggle to localize the claustrum because it is neither a basal ganglia nucleus nor part of the insular cortex, yet it sits between them as a distinct gray matter layer with tight relationships to the external and extreme capsules and their association fibers. That adjacency becomes clinically relevant in insular glioma surgery and approaches to deep lesions near the putamen, where subcortical dissection planes can drift from the extreme capsule into claustral gray or toward the external capsule and striatum. A medial perspective helps you teach and plan around capsule boundaries, the kind of landmarking that also improves MRI interpretation when a thin band of altered signal hugs the insular region in seizure-related lesions or small-vessel infarcts.
