Description
Running inferiorly from the larynx in the anterior neck, the trachea descends in the midline into the superior mediastinum, where it bifurcates at the carina into the right and left main bronchi. The right main bronchus courses more vertically and enters the hilum of the right lung, while the left main bronchus passes inferolaterally beneath the aortic arch toward the left hilum. Within each lung, the bronchi branch into lobar and segmental bronchi that track with the bronchopulmonary segments, transitioning distally to bronchioles and the terminal respiratory units within the peripheral parenchyma. Visceral pleura invests the lung surface, with the mediastinal pleura reflecting at each root. Airway orientation matters clinically. The steeper, wider right main bronchus explains why aspirated teeth, food boluses, and gastric contents more often lodge on the right, and why bronchoscopy teaching emphasizes the carina as the primary landmark for correct endotracheal tube depth. Segmental bronchial anatomy also drives thoracic surgery planning: segmentectomy and lobectomy follow bronchovascular pedicles, and radiology reports localize pneumonia, atelectasis, or an obstructing bronchogenic carcinoma by lobe or bronchopulmonary segment.
