Seen from a superior cutaway into the male cranial cavity, the posterior cerebral artery (PCA) courses posteriorly from the terminal basilar artery near the interpeduncular cistern and wraps around the lateral aspect of the midbrain toward the medial surface of the occipital lobe. Distal PCA branches fan out over the calcarine region, with smaller arterioles spreading laterally and posteriorly toward the occipital pole. Around the central vascular hub, arteries consistent with the Circle of Willis occupy a more medial position, while venous channels lie more superficially against the endocranial surface and converge toward midline dural sinuses. Bone landmarks and foramina of the cranial base frame the field, providing orientation even with the brain removed. For teaching and clinical correlation, this cutaway emphasizes how the PCA’s terminal distribution maps to visual cortex, and why an occipital infarct from a PCA occlusion classically produces contralateral homonymous hemianopia, often with macular sparing via collateral flow from the middle cerebral artery.
