Description

Arising from the dorsal midbrain and wrapping around the brainstem, the trochlear nerve (cranial nerve IV) is traced within a cutaway male skull base as it courses anterolaterally in the subarachnoid space toward the cavernous sinus. From the posterior cranial fossa it runs superior to the oculomotor nerve (CN III), then approaches the superior orbital fissure to reach the orbit and the superior oblique muscle. Surrounding bony landmarks of the cranial base are logically visible in this inferior, intracranial perspective, including the clivus, petrous part of the temporal bone, and the foramen magnum, with other cranial nerves highlighted as they align with their respective foramina. CN IV is the only cranial nerve to exit the brainstem dorsally and the thinnest motor cranial nerve, so a skull-base cutaway helps clarify why it is vulnerable to stretch. Even mild head trauma can produce trochlear palsy with vertical diplopia that worsens on looking down and in, a pattern students often confuse with oculomotor or abducens lesions unless the nerve’s long intracranial course is clear. The relationship to the tentorial edge and cavernous sinus corridor also supports teaching about skull base tumor effects and postoperative diplopia after petroclival or tentorial procedures.
