Description
Layered dissection of an adult female head and upper neck reveals the facial expression musculature overlying the viscerocranium, with the masseter occupying the lateral ramus of the mandible and the buccinator forming the muscular wall of the cheek deep to the oral commissure. Inferiorly, the sternocleidomastoid runs obliquely from the mastoid process to the manubrium and medial clavicle, framing the anterior triangle where the carotid sheath contents course. Superficial and deep vascular channels traverse the face, while cranial nerve pathways radiate from the parotid and infratemporal regions toward their target muscles and cutaneous territories. Skin and subcutaneous tissue are partially removed or rendered translucent to maintain surface landmarks while exposing the deeper anatomy. Facial and cervical anatomy is unforgiving in the operating room. The branching pattern of the facial nerve (CN VII) within and distal to the parotid gland guides parotidectomy and facelift (rhytidectomy) planes, and even small variations can explain postoperative weakness of the marginal mandibular or temporal branches. Anteromedial to the sternocleidomastoid, the common carotid artery and internal jugular vein occupy predictable relationships that matter during central venous access and carotid endarterectomy, while sensory territories of the trigeminal nerve (CN V) underpin targeted nerve blocks for laceration repair and dental or maxillofacial procedures. Small margins matter.
