Description
Arising on the dorsolateral aspect of the lateral cuneiform and running posteriorly and laterally to the dorsal surface of the cuboid, the dorsal cuneocuboid ligament spans the cuneocuboid joint as a short, flattened band of fibrous connective tissue. The cuboid sits distal to the calcaneus and lateral to the navicular, while the lateral cuneiform lies distal to the navicular and medial to the cuboid, placing this ligament squarely across the dorsal midfoot. Adjacent dorsal ligaments of the midtarsal region are suggested by the bony contours, including the dorsal calcaneocuboid and dorsal cuboideonavicular ligaments, with the bases of the fourth and fifth metatarsals positioned distal to the cuboid. Dorsal midfoot ligaments are often underappreciated until the lateral column becomes painful. A focal injury pattern is the “nutcracker” mechanism, where forefoot abduction compresses the cuboid between the calcaneus and metatarsals, stressing dorsal capsuloligamentous fibers around the cuneocuboid and calcaneocuboid articulations. This ligament is also a practical landmark when teaching stability across the transverse tarsal joint complex, separating true joint incongruity from soft tissue sprain in lateral midfoot trauma.
