Description

Running along the dorsoulnar forearm, the extensor digiti minimi (EDM) muscle belly tapers into a distinct tendon that passes posterior to the distal radius and ulna beneath the extensor retinaculum before continuing onto the dorsum of the hand. Distal to the wrist, the EDM tendon tracks toward the fifth metacarpal, lying ulnar to the extensor digitorum tendon slips and superficial to the dorsal interossei and metacarpals. Dorsal veins and superficial branches of the ulnar nerve are suggested in the subcutaneous plane. Clear landmarks. Isolating EDM from a posterior perspective matters because it clarifies the independent extension of the little finger at the metacarpophalangeal joint and the tendon’s relationship to the extensor compartments, a common point of confusion when students first learn dorsal wrist anatomy. Tendon irritation can occur where EDM traverses under the extensor retinaculum in the fifth extensor compartment, and the dorsal ulnar region is also where clinicians assess for extensor tendon subluxation or synovitis in inflammatory arthropathies. Hand surgeons pay attention to this tendon as a donor in tendon transfer and reconstruction, since its excursion and line of pull often remain reliable when adjacent extensor digitorum slips are compromised.
