Description
Anterior orientation of the right brachium centers the humerus from the glenohumeral joint to the elbow, with the humeral head articulating with the glenoid cavity of the scapula and the shaft descending to the distal condyles. Proximally, the lesser tubercle sits on the anteromedial aspect of the proximal humerus, just medial to the intertubercular (bicipital) groove, and it is positioned inferior to the humeral head and medial to the greater tubercle. The fracture line interrupts the cortical contour of the proximal humerus near this region, while the radius and ulna align distally at the humeroulnar and humeroradial articulations. Fracture location relative to the lesser tubercle matters because this landmark marks the insertion of subscapularis, so displacement can reflect rotator cuff pull and alter internal rotation mechanics at the shoulder. In proximal humerus fractures, especially those involving the surgical neck, anterior views help correlate bony fragments with the course of the long head of biceps tendon in the bicipital groove and with nearby neurovascular structures, including the axillary nerve and posterior circumflex humeral artery around the proximal humerus. Misjudging fragment orientation here complicates open reduction and internal fixation and can contribute to malunion or persistent anterior shoulder pain.
