Description
Seen from a posterior perspective, the proximal radius is shown with a fracture at the radial neck producing clear malalignment between the radial head and the shaft, with the head remaining closest to the elbow joint surface while the diaphysis deviates distally. The ulna lies medial to the radius along the forearm, providing a stable reference for the degree of angulation and any relative displacement at the proximal radioulnar relationship. Bony landmarks you would expect in this projection include the radial head and neck superiorly, the posterior contour of the radial tuberosity just distal to the neck, and the interosseous border running distally toward the wrist. Posterior visualization of a radial head or neck fracture matters because subtle angulation can be missed when attention stays on the radiocapitellar joint line alone, yet even modest displacement can block pronation and supination and alter load transfer through the lateral column of the elbow. Radial neck fractures in adult men often follow a fall on an outstretched hand, and assessment must consider associated injuries such as an Essex-Lopresti pattern (radial head/neck injury with interosseous membrane disruption and distal radioulnar instability) or a Monteggia-variant mechanism when ulna alignment is abnormal. Nerve proximity also makes this level unforgiving, since the posterior interosseous nerve courses near the supinator around the proximal radius. Alignment drives management.
