Seen in section through the male cranium, the calvaria and skull base form the anterior, middle, and posterior cranial fossae as a pale bony envelope around the central brainstem. The midbrain lies superior to the pons, with the medulla oblongata continuing inferiorly toward the foramen magnum, while multiple cranial nerves emerge from the ventral and lateral brainstem and course anterolaterally toward their respective foramina. Along the middle cranial fossa, the petrous part of the temporal bone and adjacent sphenoid contours sit lateral to the clivus, shaping the corridors that funnel nerves and vessels out of the cranial cavity. Orientation of the brainstem to the cranial fossae is the operative and diagnostic map for skull base disease. A few millimeters matter: trigeminal nerve trajectories toward Meckel’s cave and the foramina ovale and rotundum, the facial and vestibulocochlear bundle toward the internal acoustic meatus, and the lower cranial nerves toward the jugular foramen and hypoglossal canal. These relationships anchor interpretation of cranial neuropathies and localize lesions such as vestibular schwannoma, meningioma along the petrous ridge, or jugular foramen paraganglioma, and they parallel the corridors used in retrosigmoid and transpetrosal approaches.
