Description
Centered on the ventral surface of the brainstem, the basilar artery ascends in the midline along the basilar sulcus of the pons after the paired vertebral arteries unite at the pontomedullary junction. From its lateral aspect arise the anterior inferior cerebellar arteries, and more superiorly the superior cerebellar arteries, which sweep posterolaterally toward the cerebellar hemispheres, while smaller pontine branches perforate directly into the pons. At the rostral end, the basilar bifurcates into the posterior cerebral arteries just inferior to the diencephalon, near the posterior clinoid region and the tentorial edge. Short and sharp. Surgeons and neurointerventionalists care about this corridor because small shifts in anatomic relationships translate into brainstem ischemia. Basilar artery occlusion produces posterior circulation stroke with coma, quadriplegia, and locked-in syndrome, and the perforator-rich segment at the mid-pons is where thrombectomy carries the highest risk of devastating infarct. An overhead cranial cavity view also helps orient the basilar apex to adjacent landmarks used in aneurysm planning, including the posterior cerebral arteries, superior cerebellar arteries, and the venous sinuses that frame operative and endovascular trajectories.
