Description
Posterior surface anatomy of an obese adult Black male is shown with a semi-transparent anatomical overlay of the upper gastrointestinal tract. The thoracic esophagus descends in the midline and passes through the esophageal hiatus to join the stomach at the cardia, with the rounded fundus positioned superiorly beneath the left hemidiaphragm and the body of the stomach extending inferiorly toward the pyloric region. Distal to the pylorus, the proximal duodenum and an initial segment of small intestine are indicated, with the greater curvature sweeping inferolaterally to the left compared with the more medial lesser curvature. A posterior, slightly oblique orientation clarifies how the stomach sits high in the left upper quadrant even when abdominal adiposity obscures external landmarks. That matters when teaching referred pain patterns (for example, posterior gastric or duodenal ulcer pain that can radiate to the back) and when planning upper endoscopy, bariatric surgery, or laparoscopic port placement where the gastroesophageal junction and fundus must be approached with an accurate mental map of depth and tilt.
