Description

Visible in the retroperitoneum, the paired kidneys sit high in the posterior abdomen on either side of the vertebral column, the right kidney positioned slightly inferior to the left. From each renal hilum, the renal pelvis narrows into a ureter that descends anterior to the psoas major, crossing the pelvic brim to enter the urinary bladder at its posterolateral angles. The bladder occupies the anterior pelvis, posterior to the pubic symphysis and anterior to the vagina and cervix, and the short female urethra courses inferiorly from the bladder neck to the external urethral orifice in the vestibule. Female pelvic topography is clear. This arrangement matters when you are teaching or planning around the three physiologic ureteric narrowings where calculi commonly lodge: the ureteropelvic junction, the crossing of the iliac vessels at the pelvic brim, and the ureterovesical junction. The close relationship of the distal ureter to the uterine artery (water under the bridge) is a constant surgical hazard during hysterectomy and pelvic lymphadenectomy, and this view supports that spatial reasoning better than isolated organ diagrams.
