Arising from the ischial portion of the acetabular rim, the ischiofemoral ligament fans superolaterally across the posterior hip joint capsule toward the femoral neck and the base of the greater trochanter. From a posterior perspective, its fibers lie deep to the gluteal musculature (removed here) and sit lateral to the sacrum and coccyx, spanning between the posterior pelvis and the proximal femur. Bilateral hip joints are framed by the ilium superiorly and the ischium inferiorly, with the femoral heads seated in the acetabula and the capsule rendered as a highlighted band around the joint margins. Orientation is unambiguous. Posterior capsular anatomy matters because it is the primary soft tissue checkrein against excessive internal rotation and extension at the hip, a relationship you can teach cleanly when the ischiofemoral fibers are traced from the ischium to the femur.
