Description
Running in a lateral perspective along the posterior thoracic wall, the spinal nerves exit the intervertebral foramina and quickly divide into dorsal and ventral rami adjacent to the vertebral column. The dorsal rami course posteriorly across the transverse processes and then split into medial and lateral branches, with the lateral branches tracking more laterally toward the costal angles and the superficial back. Ribs sweep anteriorly from their thoracic vertebrae, while the intervertebral discs stack between vertebral bodies and the sacrum and coccyx anchor the inferior column. Color coding highlights the nerve pathways against the bony landmarks. Lateral branches of the dorsal rami matter because they supply segmental motor innervation to intrinsic back muscles and carry cutaneous sensation from the posterolateral trunk, territories that are commonly tested when localizing radiculopathy versus peripheral nerve injury. This view helps clarify where dorsal rami become distinct from ventral rami before the ventral rami continue as intercostal nerves, and it places the branching pattern in direct relation to the costotransverse joints and the rib cage, a frequent reference during posterior thoracic approaches. Small nerves, big consequences. Irritation or entrapment of dorsal ramus branches, including the lateral cutaneous pathways, can present as focal paraspinal or flank pain that differs from intercostal neuralgia.
