Description
Oriented to a posterior view of the right knee, the lateral collateral ligament (fibular collateral ligament) is shown as a distinct fibrous band spanning from the lateral femoral epicondyle to the head of the fibula. The ligament runs inferolaterally along the posterolateral aspect of the joint, remaining superficial to the popliteus tendon and clearly separated from the fibrous joint capsule. Adjacent bony landmarks include the distal femur superiorly and the proximal tibia and fibula inferiorly, with the tibia lying medial to the fibula in standard anatomical position. Clean separation from the capsule stands out. That capsular independence matters clinically because the LCL behaves differently from the medial collateral ligament, which blends with the medial capsule and meniscus. Varus stress testing at 0 degrees and 30 degrees of flexion targets LCL integrity, and posterolateral corner injuries often combine LCL disruption with injury to the popliteus complex and popliteofibular ligament, producing rotary instability that can be missed if you only think in terms of cruciate ligaments. Surgical reconstruction planning also relies on recognizing the femoral origin near the lateral epicondyle and the fibular insertion near the fibular styloid, where the common fibular nerve lies at risk just distal and posterior to the fibular head.
