Description
Positioned on the lateral aspect of the male knee, the lateral patellar retinaculum forms a dense fibrous expansion bridging from the lateral border of the patella into the adjacent deep fascia and capsular tissues. From this perspective, the patella sits anterior to the distal femur and superior to the tibial plateau, with the articular cartilage lining the femoral trochlea and posterior surface of the patella. Lateral stabilizers, including the lateral collateral ligament running from the lateral femoral epicondyle to the fibular head, lie posterior and slightly inferior to the retinacular fibers. Clear layering matters. Clinically, the lateral retinaculum is central to patellofemoral mechanics because excessive lateral tethering can drive lateral patellar tilt and contribute to anterior knee pain, lateral facet chondrosis, and recurrent maltracking. Orthopaedic examination maneuvers and imaging interpretation often hinge on appreciating how the retinaculum relates to the patella, trochlear groove, and lateral soft tissues, and why a tight lateral restraint can overpower the medial patellofemoral ligament after a dislocation event. Surgical decisions such as lateral retinacular lengthening or selective release demand that anatomy in this region be understood as capsuloligamentous tissue, not a single strap.
