Description
Arising from the pulmonary trunk, the left pulmonary artery courses leftward and slightly posterior as it passes under the aortic arch toward the hilum of the left lung, where it lies superior to the left main bronchus. From this anterior, radiograph-like perspective, the vessel’s lobar branches can be followed laterally into the upper and lower lobes, set against semi-transparent lung parenchyma and the branching trachea and bronchi. Pulmonary veins return medially toward the left atrium and sit more inferior and anterior at the hilum, helping distinguish arterial from venous pathways. Orientation is immediate. Anterior visualization of the left pulmonary arterial tree matters because it mirrors how clinicians first encounter it on chest radiographs, CT angiography reconstructions, and catheter-based pulmonary angiography. Enlargement or abrupt cutoff of a left pulmonary artery branch raises concern for pulmonary embolism, while a prominent left hilar artery can be a clue to pulmonary hypertension or congenital shunt physiology. The relationship to the left main bronchus and peribronchial nodes also frames common causes of extrinsic vascular narrowing, including left hilar malignancy or fibrosing mediastinitis.
