Levator Anguli Oris Muscle Under the Skin, Anterior View

Description

Emerging from the canine fossa of the maxilla, the levator anguli oris (caninus) runs inferiorly and slightly laterally to converge at the modiolus at the corner of the mouth. From an anterior, subcutaneous perspective in a male face, its slender belly is seen deep to the zygomaticus minor and lateral to the levator labii superioris, with orbicularis oris forming a circular sling around the oral fissure. Superiorly, orbicularis oculi frames the palpebral fissure, while the buccinator and masseter occupy the lateral cheek and mandibular ramus region. Bilateral symmetry is maintained. This is a modiolus-centered view. The levator anguli oris is easy to confuse with the elevators of the upper lip in dissection and in operative planning, but its insertion at the mouth angle makes it a key vector for elevating the commissure during smiling and for balancing perioral tone. Clinically, its course and depth matter in approaches to the nasolabial fold, repair of commissure lacerations, and during chemodenervation or filler placement where misplacement can flatten the smile or accentuate asymmetry; nearby branches of the facial nerve to the upper lip and cheek are also at risk in superficial planes.
