Description

Anterior anatomy of the pediatric thorax is rendered with the trachea descending in the midline of the neck to the thoracic inlet, where it bifurcates at the carina into the right and left main bronchi. Each bronchus courses inferolaterally into its lung hilum, with the right lung broader and the left lung narrower to accommodate the cardiac impression along its medial border. Lobation is defined within the thoracic cavity, with the right superior, middle, and inferior lobes separated by horizontal and oblique fissures, and the left superior and inferior lobes divided by an oblique fissure. Clear orientation. This view supports teaching of pediatric airway caliber and branching, where small decreases in luminal diameter translate into large increases in airflow resistance, a practical point when discussing bronchiolitis or asthma exacerbations in children. The midline trachea and symmetric main bronchi also provide a baseline for recognizing pathologic deviation, as in tension pneumothorax or large pleural effusion, and for explaining why aspirated foreign bodies more often lodge in the more vertical right main bronchus.
