Description
Presented in anterior anatomical position, the larynx continues inferiorly as the trachea with its C-shaped cartilaginous rings, bifurcating at the carina into the right and left main bronchi. Each bronchus enters the lung at the hilum, where the proximal bronchial tree branches into lobar and segmental bronchi that course laterally within the parenchyma. The right lung sits slightly more inferior at its base and displays a focal, dark malignant mass within the anterior aspect of the parenchyma, separated from the mediastinal midline structures by the mediastinal pleura and the proximal bronchial pathways. Small, localized disease. Stage 1 lung cancer is defined by a tumor confined to the lung without regional lymph node involvement, so an anterior view that keeps the trachea, carina, and proximal bronchi in frame supports staging discussions and procedural planning. Lesion position relative to the hilum matters when deciding between wedge resection, segmentectomy, or lobectomy, and it also predicts whether bronchoscopy is likely to yield an endobronchial target or whether CT-guided percutaneous biopsy is a better route. For teaching, it reinforces why even small peripheral tumors can be clinically silent until found on screening CT, yet still demand careful attention to margins and nodal sampling (stations 2R/2L, 4R/4L, and 7) at surgery.
