Description
Posterior thoracic anatomy of a male child is rendered with the integument semi-transparent, centering the trachea as it descends in the midline and bifurcates at the carina into the right and left main bronchi. The right lung occupies the right hemithorax and is partitioned into superior, middle, and inferior lobes by the horizontal and oblique fissures, while the left lung lies slightly more medial toward the cardiac notch region and is divided into superior and inferior lobes by a single oblique fissure. Laterally, the lungs contour to the inner rib cage, and posteriorly they overlie the thoracic vertebral column; the esophagus tracks inferiorly just posterior to the trachea within the posterior mediastinum. Bronchial arborization is suggested toward the periphery, indicating the pathway to bronchioles and alveoli. A posterior perspective is the cleanest way to teach lobar geography and fissure orientation without the distraction of the sternum and anterior mediastinal structures, and it aligns with how the lungs are assessed on the back during physical examination. Percussion and auscultation landmarks for pediatric pneumonia map more directly to the posterior basal segments, where dependent atelectasis and lower lobe consolidation often collect in a supine child. That trachea-esophagus relationship matters too, since esophageal foreign body impaction can present with cough or wheeze by compressing the posterior membranous trachea.
