Description
Viewed from superiorly into the cranial cavity of an adult male skull, the anterior, middle, and posterior cranial fossae are laid out in sequence from anterior to posterior, separated by the lesser wings of the sphenoid and the petrous ridges of the temporal bones. Centrally, the body of the sphenoid forms the sella turcica and hypophyseal fossa, with the clinoid processes bracketing the expected course of the cavernous segment of the internal carotid arteries. Posteriorly, the foramen magnum sits in the occipital bone, with the basilar part anterior to it and the occipital condyles positioned inferolaterally at its margins. Bony landmarks dominate. Clinical teaching often hinges on this top-down skull base view because it ties neurovascular pathways to fixed foramina: optic canal, superior orbital fissure, foramen rotundum, ovale, and spinosum across the middle cranial fossa, then the internal acoustic meatus and jugular foramen along the posterior fossa. Surgical corridors for transsphenoidal hypophysectomy orient around the tuberculum sellae and dorsum sellae, while skull base fractures across the petrous temporal bone raise immediate concern for carotid canal injury, CSF otorrhea, and cranial nerve VII and VIII deficits. Venous sinus grooves and arterial impressions also map common sites of epidural hematoma and dural-based lesions. Anatomy with consequences.
