Description
Running along the medial aspect of the knee, the medial collateral ligament (MCL), tibial (medial) collateral ligament, appears as a broad longitudinal band spanning from the medial femoral epicondyle to the medial surface of the proximal tibia. Superiorly it lies adjacent to the adductor tubercle region, then descends distally and slightly anteriorly toward the anteromedial tibia, superficial to the joint capsule. The medial tibial condyle and medial femoral condyle frame the ligament deep to it, and the medial joint line sits immediately posterior to the anterior fibers. Clear orientation. A medial perspective matters because it clarifies the layered anatomy that governs valgus stability, with the superficial MCL readily distinguished from deeper capsular structures that blend with the medial meniscus. This is the view most aligned with clinical grading of MCL injury after a valgus stress mechanism, where tenderness localizes along the femoral attachment or the tibial insertion and laxity is tested at 30 degrees of knee flexion. It also supports surgical teaching around medial approaches, where incisions near the medial epicondyle must respect the saphenous nerve and the sartorial fascia while exposing the MCL for repair or augmentation.
