Description
Medial occipitotemporal gyrus (often termed the lingual gyrus) occupies the ventromedial surface of the posterior cerebral hemisphere in this inferior perspective, extending longitudinally from the occipital pole toward the posterior temporal region. The gyrus is bounded laterally by the collateral sulcus and medially by the rhinal sulcus, with the sulcal walls defining a narrow, elongated cortical fold on the basal surface. Posteriorly, the occipital portion broadens near the inferomedial occipital cortex, while more anteriorly the gyrus tapers as the basal temporal surface transitions toward parahippocampal territory. Sulcal relationships are the point. Basal temporal anatomy matters because small differences in sulcal patterning change how clinicians describe lesion location and plan approaches. Radiologists often localize ischemia in the posterior cerebral artery territory to the lingual gyrus when correlating inferior temporal complaints with MRI, and neurosurgeons use the collateral and rhinal sulci as surface landmarks when planning stereo-EEG trajectories or resections for drug-resistant temporal lobe epilepsy to avoid unnecessary violation of adjacent cortex.
