Description
Lateral left-sided cardiac anatomy is centered on the mitral (left atrioventricular) valve at the junction of the left atrium and left ventricle. The anterior and posterior mitral leaflets attach circumferentially to the mitral annulus and project inferiorly into the ventricular cavity, where chordae tendineae descend to the anterolateral and posteromedial papillary muscles. Fibrous cords span from the free margins of the leaflets to the papillary tips, lying deep to the ventricular endocardium and framed externally by the left ventricular myocardium and its layered muscle architecture. A clean spatial story. Mitral valve competence depends on this subvalvular apparatus, so a lateral perspective helps you track leaflet, chordal, and papillary alignment along the long axis of the left ventricle. Chordal rupture after ischemia of the posteromedial papillary muscle (single coronary supply in many hearts) can precipitate acute severe mitral regurgitation, with leaflet flail toward the left atrium and rapid pulmonary edema. The same relationships matter in mitral valve repair, where surgeons preserve or replace chordae to maintain left ventricular geometry and avoid postoperative systolic dysfunction.
