Description
Presented in sagittal section, the male heart is opened to expose the left atrium superiorly and posteriorly as it communicates with the thick walled left ventricle through the bicuspid (mitral) valve. Two mitral cusps form the atrioventricular orifice, their free margins directed inferiorly into the ventricular cavity, while their basal attachments meet the fibrous annulus in the atrioventricular junction. Chordae tendineae descend from the cusp edges to the anterolateral and posteromedial papillary muscles on the ventricular wall, positioned to resist systolic prolapse toward the atrium. Portions of the right ventricle and the interventricular septum are also visible, providing an anterior reference for the left sided inflow tract. A sagittal cut gives a clean read on the left ventricular inflow and the subvalvular apparatus, the anatomy that underlies mitral regurgitation from chordal rupture, papillary muscle dysfunction after inferior myocardial infarction, or leaflet prolapse in myxomatous degeneration. This is also the view that helps correlate gross anatomy with echocardiographic long axis assessment, where leaflet coaptation and annular motion are judged relative to the left ventricular outflow tract and aortic root. Coaptation is the point.
