Description

Centered in the anterior thorax, the heart sits within the middle mediastinum, with the right atrium and right ventricle positioned anterolaterally and the left ventricle forming the left inferolateral cardiac border. Laterally, the right and left lungs flank the pericardial sac, their medial surfaces approaching the cardiac silhouette and leaving a narrow cardiomediastinal interface where the great vessels course. Superiorly, the trachea descends in the neck and bifurcates at the carina into the main bronchi, which pass inferolaterally into the hila alongside the pulmonary arteries and veins, while the ascending aorta and pulmonary trunk emerge from the ventricular outflow tracts and continue toward the superior mediastinum. Understanding the anatomic proximity of bronchi, pulmonary vessels, and cardiac chambers at the hila and mediastinum matters when you are explaining why a left atrial enlargement can elevate the left main bronchus or why hilar lymphadenopathy can narrow a bronchus and alter ventilation. The anterior standing view also maps cleanly to frontal chest radiography, where the right heart border is largely right atrium, the left heart border is largely left ventricle, and the pulmonary outflow tract and aortic knob create predictable contours that shift with pulmonary hypertension or aortic aneurysm.
