Description
Posterior trunk anatomy is rendered with the thoracolumbar fascia highlighted as a broad aponeurotic sheath spanning the thoracic and lumbar back into the upper gluteal region. Superiorly, the fascia blends with the posterior thoracic wall deep to the trapezius and continues inferiorly toward the sacrum and posterior iliac crest, lying deep to latissimus dorsi and superficial to the intrinsic back muscles in the lumbar region. Laterally, its laminae course toward the abdominal wall, while medially it anchors along the lumbar spinous processes and adjacent vertebral elements. Fiber direction is indicated in the overlying trapezius, latissimus dorsi, and gluteus maximus, with portions of the shoulder and arm showing deltoid and triceps brachii for orientation. A key landmark. For teaching lumbar mechanics, few regions repay attention like the thoracolumbar fascia because it forms a tensioned linkage between latissimus dorsi, gluteus maximus, and the lumbar spine via its attachments to the lumbar vertebrae, sacrum, and iliac crest. This configuration matters in low back pain models that emphasize load transfer across the lumbopelvic junction, and it also frames common regional anesthesia and procedural anatomy, including ultrasound guided lumbar interfascial plane blocks (for example, the thoracolumbar interfascial plane block) that target fascial planes adjacent to the erector spinae. Surgeons also encounter it during posterior approaches to the lumbar spine and during harvesting or mobilization of paraspinal muscle flaps.
