Description
Posteriorly, the obturator nerve is traced from the lumbar plexus within the psoas major, then along the lateral wall of the small pelvis toward the obturator canal at the superior margin of the obturator foramen. The nerve courses inferior and anterior to the pelvic brim, approaching the obturator internus and its fascia, with the hip bones (ilium, ischium, pubis) providing clear bony reference points. More laterally and inferiorly, the proximal femur and patella anchor the lower limb orientation, while the sciatic nerve descends in the gluteal region and posterior thigh as a contrasting major trunk. Blue venous channels accompany the limb pathways and help establish the neurovascular geography. A posterior perspective is practical because it clarifies relationships that matter during pelvic and hip surgery, where the obturator nerve can be injured near the obturator canal or compressed by pelvic hematoma. It also aligns with common pain patterns, obturator neuropathy often presents with medial thigh pain and weakness of hip adduction, findings that can be mistaken for lumbar radiculopathy unless the clinician recalls the nerve’s pelvic course and exit point.
