Description
Pontocerebellum (neocerebellum) dominates the superior surface of the human cerebellum, with the paired cerebellar hemispheres expanding laterally away from the midline vermis. Along this fixed superior perspective, the hemispheric lobes form broad convex contours, while the vermis remains a narrower, median strip separating right and left folia. Shallow primary and secondary fissures score the cerebellar cortex, organizing the surface into lobules that align with the posterior lobe, including the lobulus simplex and the adjacent hemispheric expansions often grouped as Crus I and Crus II. Fine foliation is visible across the cortex. Clinical relevance centers on the pontocerebellar loop that gives this region its name: corticopontine fibers synapse in the pontine nuclei, cross, and enter the cerebellum through the middle cerebellar peduncles to influence lateral hemisphere planning of movement rather than midline posture. Lateral cerebellar injury here classically produces ipsilateral limb ataxia, dysmetria, and intention tremor, findings that differ from vermian lesions associated with truncal ataxia.
