Description
Seen from a lateral aspect of the male lower limb, the posterior femoral cutaneous nerve descends from the sacral plexus at the posterior pelvis and tracks inferiorly deep to the fascia along the posterior thigh. Proximally it lies medial and posterior to the greater trochanter region, then runs superficially relative to the hamstring muscle mass while remaining posterior to the femoral shaft. The nerve parallels the course of the sciatic nerve in the proximal thigh but stays more cutaneous in its distribution as it approaches the popliteal fossa. Clear bony landmarks include the pelvis, femur, patella, tibia, fibula, and the tarsals, metatarsals, and phalanges distally. Lateral presentation matters because it clarifies the nerve’s relationship to fascial planes and to common access corridors in posterior thigh procedures. Entrapment or irritation of the posterior femoral cutaneous nerve can contribute to posterior thigh or gluteal pain syndromes and may be confused clinically with proximal hamstring tendinopathy or sciatica, so distinguishing its course relative to the sciatic nerve helps frame a focused differential. This is also a practical orientation for teaching why deep gluteal injections should target the superolateral quadrant to reduce the risk of injuring nearby neural structures. Use this illustration for gross anatomy teaching on the sacral plexus and cutaneous innervation of the thigh, for pain medicine or sports medicine lectures on posterior thigh dysesthesia, and for surgical education materials covering posterior approaches to the hip and thigh where fascial dissection planes matter.
