Description
Principal sensory nucleus of the trigeminal nerve is positioned in the dorsal pontine tegmentum, deep to the posterior surface of the brainstem and medial to the trigeminal root entry zone at mid-pons. From this posterior perspective, the nucleus sits superior to the spinal trigeminal nucleus and tract, while remaining lateral to midline tegmental structures that approach the floor of the fourth ventricle. Nearby brainstem landmarks that commonly frame it in teaching include the motor nucleus of V (more medial) and the mesencephalic trigeminal nucleus extending rostrally toward the upper pons. Orientation is posterior and human. Discriminative touch, pressure, and vibration from the face synapse in this cell group before second-order neurons decussate and ascend as the trigeminothalamic tract toward the ventral posteromedial (VPM) nucleus of the thalamus, so focal pontine lesions can produce ipsilateral facial hypoesthesia with preserved pain and temperature depending on involvement of the spinal trigeminal system. This is clinical anatomy you can localize at the bedside. A dorsal brainstem emphasis helps clarify how the principal sensory nucleus relates to the trigeminal nerve’s pons entry zone and to adjacent tegmental nuclei that are often conflated on simplified diagrams. Use this illustration in neuroanatomy and cranial nerve modules to anchor trigeminal sensory pathways, or in neurology and stroke education to support lesion localization in dorsolateral pontine infarcts, demyelinating plaques, or intrinsic pontine tumors that encroach on trigeminal nuclei.
