Running along the anterior lumbar spine, the psoas minor appears as a slender, flattened muscle belly lying superficial to the bulk of psoas major, with its fibers oriented inferiorly toward the pelvic brim. Superiorly, it relates to the lower thoracic and upper lumbar vertebral bodies and the adjacent intervertebral discs, while laterally it remains close to the medial margin of psoas major rather than the iliacus in the iliac fossa. Inferiorly, its tendon tracks toward the iliopubic eminence and pecten pubis, passing anterior to the hip joint as the femoral head sits deep within the acetabulum. Small muscle. Big landmark. Because psoas minor is absent in a sizable proportion of individuals, an anterior view that separates it from psoas major helps prevent mislabeling on exams and in atlases, and it clarifies why isolated psoas minor strain is uncommon compared with iliopsoas pathology.
