Description
Viewed from the right lateral thorax, the right atrium and right ventricle sit anteroinferior to the right lung and rest directly on the central tendon of the diaphragm through the fibrous pericardium. Superiorly, the great venous inflow to the right atrium aligns with the superior vena cava, while the inferior vena cava approaches the atriocaval junction as it traverses the diaphragmatic caval opening. Laterally, the mediastinal pleura and the right lung, with its oblique and horizontal fissures separating superior, middle, and inferior lobes, frame the pericardial sac. A short segment of trachea and proximal bronchial airway may be appreciated superior to the hilum. Pericardium-on-diaphragm anatomy matters because the two structures move together during respiration and because pericardial pain can refer along the phrenic nerve (C3 to C5) as it descends on the fibrous pericardium toward the diaphragm. Small changes in diaphragmatic position alter the cardiophrenic angle and can mimic or mask pathology on chest radiographs, including pericardial effusion, right lower lobe atelectasis, or an elevated hemidiaphragm from phrenic nerve palsy. A practical landmark. The lateral relationship also grounds procedural planning for subxiphoid pericardiocentesis and for thoracoscopic pericardial window creation, where avoiding the phrenic nerve course along the pericardium is the difference between drainage and postoperative diaphragmatic dysfunction.
