Description
Arising from the posterior neck, the occipital artery courses superiorly along the occipital bone and fans out into multiple scalp branches that arborize toward the vertex, running superficial to the galea aponeurotica and deep to the skin. Laterally, posterior auricular arterial branches track behind the auricle toward the mastoid region, while midline perforators cross the nuchal fascia near the external occipital protuberance. A venous companion network parallels the arteries and drains inferiorly toward the suboccipital and external jugular pathways. Dense collateral channels are evident. Posterior scalp vascularization matters most when you are planning incisions and flap elevation across the occiput, because the occipital artery is the dominant pedicle for many posterior scalp flaps and it anastomoses broadly with the superficial temporal and contralateral occipital systems. Lacerations over the galea can bleed briskly when these branches retract, and iatrogenic injury during subgaleal dissection can compromise wound edges. Occipital artery–based anatomy also underpins targeted occipital nerve blocks and decompression approaches for occipital neuralgia, where vessel-nerve proximity can explain pulsatile trigger points.
