Description
Posterior cervical anatomy is centered on the semispinalis capitis muscle, shown bilaterally as it ascends from the upper thoracic and lower cervical region to the occipital bone between the superior and inferior nuchal lines. Its muscle bellies lie deep to splenius capitis and trapezius, running superiorly and slightly medially over the cervical vertebrae, with the spinous processes forming a midline bony reference. Lateral to the cervical column, the scapulae frame the upper back, while the posterior arches and upper ribs provide inferior context for the cervicothoracic junction. Semispinalis capitis is a principal extensor of the head and a contributor to contralateral rotation, so this posterior view is the teaching standard for separating the deep transversospinalis layer from the superficial splenius group. That distinction matters in posterior cervical approaches, where layered dissection and retraction can denervate or devascularize the extensor mass and contribute to postoperative axial neck pain.
