Description
Centered on the anterior abdominal skeleton, the lower thoracic cage transitions into the lumbar spine, with the costal margins of ribs 7 to 10 framing the superior field and ribs 11 and 12 tapering inferolaterally toward the flank. Inferior to L1 through L5, the sacrum sits in the midline and articulates with the right and left ilia at the sacroiliac joints, completing the posterior component of the pelvic ring. The paired hip bones are read as ilium superiorly, ischium posteroinferiorly, and pubis anteromedially, meeting at the pubic symphysis and defining the pelvic inlet. Landmarks are clear. For teaching, this anterior alignment clarifies how load transfers from lumbar vertebral bodies through the lumbosacral junction into the sacroiliac complex and onward to the acetabula. It also maps cleanly onto common clinical problems: anterior disc height loss and osteophyte formation in the lower lumbar spine, pain referral patterns near L4 to L5 and L5 to S1, and pelvic ring instability where the pubic symphysis and sacroiliac joints behave as a functional unit. Obstetric and gynecologic anatomy often begins with these bony boundaries, because pelvic inlet shape and sacral promontory position influence engagement, station, and operative approach.
