Description
Cerebellar uvula (lobulus IX of the vermis) occupies the inferior midline surface of the cerebellum, expressed as tightly packed folia of cerebellar cortex running transversely across the vermian ridge. Laterally, the uvula is flanked by the cerebellar tonsils, with the intervening sulci marking the paravermian transition toward the hemispheric inferior lobules. A prepyramidal fissure separates it superiorly from the pyramid, while the uvulonodular fissure defines its anterior relationship to the nodulus near the roof of the fourth ventricle. Midline anatomy matters. Clinically, the uvula and adjacent nodulus sit in the vestibulocerebellar network, so focal pathology here can present with truncal ataxia, impaired stance control, and central vestibular signs such as gaze-evoked nystagmus rather than the limb dysmetria typical of hemispheric lesions. The inferior vermian perspective also orients the viewer to the crowded topography of the posterior fossa, where tonsillar descent in Chiari I malformation and postoperative swelling can distort the midline vermis and narrow the cisterna magna, changing surgical corridors and complicating interpretation of posterior fossa imaging. Consistent folial lighting and crisp fissural edges help separate true lobular boundaries from surface shadowing.
