Sulcus Terminalis of the Heart

Description

Running vertically along the right cardiac border, the sulcus terminalis forms the external groove that marks the junction between the smooth sinus venarum of the right atrium and the trabeculated right atrial appendage (auricle). Superiorly it approaches the entry of the superior vena cava, and inferiorly it descends toward the inferior vena cava and the right atrioventricular (coronary) sulcus. An anterior thoracic perspective also places the right ventricle anteroinferior to the atria, the aortic arch curving superiorly and leftward, and the pulmonary trunk anterior to the ascending aorta, with epicardial coronary arteries and veins crossing the atrioventricular and interventricular grooves. Clinically, the sulcus terminalis matters because it mirrors the internal crista terminalis, a landmark used to orient catheter and lead placement within the right atrium. Electrophysiology mapping often references this region when distinguishing smooth-walled sinus venarum from the pectinate muscle territory of the auricle, and when navigating near the cavoatrial junction where atrial tachyarrhythmia circuits and ablation lines may be planned. A clear external cue also helps correlate surface anatomy with the sinoatrial node’s usual location near the superior end of the crista terminalis at the SVC-RA junction. Small landmark, big consequences.
