Description
Anterior neck anatomy is presented in a pediatric male, with the thyroid gland highlighted as paired right and left lobes connected by a midline isthmus that crosses the anterior trachea just inferior to the laryngeal prominence. Superiorly, the lobes reach toward the thyroid cartilage and inferiorly taper toward the thoracic inlet, hugging the anterolateral surfaces of the upper tracheal rings. Small parathyroid glands are implied on the posterior aspect of each lobe, though they sit deep to the thyroid capsule and are not typically prominent from a strict anterior perspective. Larynx and trachea anchor the midline. Pediatric thyroid anatomy matters because size, position, and surface landmarks differ from adults, and those differences affect both examination and intervention. A small goiter, thyroglossal duct cyst moving with swallowing, or anterior neck mass can shift the relationship between the thyroid isthmus and the larynx, and ultrasound interpretation relies on knowing where the gland normally sits relative to the thyroid cartilage and trachea. Surgeons also work close to structures at risk, including the recurrent laryngeal nerves near the tracheoesophageal grooves and the parathyroids on the posterior thyroid, with postoperative hypocalcemia and voice change being specific complications of thyroidectomy. Palpation starts here.
