Description
Three-dimensional rendering centers on the vaginal mucosa and its rugae (plicae vaginales), with transverse folds arranged around the lumen and organized into anterior and posterior columns of rugae (columnae rugarum). Superiorly, the rugae diminish toward the vaginal fornices abutting the cervix of the uterus, while inferiorly the folds approach the vaginal vestibule. The corrugations sit on the lamina propria overlying the muscularis, creating a pleated surface that expands radially and longitudinally. Teaching the rugae in 3D matters because learners often confuse mucosal folding with fixed septa or pathologic ridging on speculum exam. This surface architecture explains both distensibility in intercourse and childbirth and the way inflammation or hypoestrogenism can change the exam, with postmenopausal atrophic vaginitis flattening rugae and increasing mucosal friability and dyspareunia. Clear columnar orientation also supports procedural anatomy, since instrumentation and tampon placement follow the canal’s long axis while the rugae influence friction and retention.
