Description
Cutaway anatomy of the adult female pelvis frames the urinary bladder in the midline, inferior to the peritoneal reflection and anterior to the uterus and upper vagina. The bladder wall is rendered in layers, with urothelium and lamina propria lining the lumen, muscularis propria (detrusor muscle) forming the thick middle coat, and perivesical fat external to the serosa/adventitia. Distally, the bladder neck funnels into the proximal urethra, while the ureteric orifices enter posterolaterally at the trigone, a key landmark when mapping multifocal urothelial carcinoma. Progression is organized around the TNM system, separating non muscle invasive disease (Ta, carcinoma in situ, and T1 invasion into lamina propria) from muscle invasive carcinoma (T2 extension into detrusor) and extravesical spread (T3 into perivesical tissue). Once tumor breaches the bladder wall, the anatomic neighbors matter, invasion can track anteriorly toward the retropubic space or posteriorly into the anterior vaginal wall and cervix, while advanced T4 disease may reach the pelvic wall. Nodal pathways are not an afterthought, they follow predictable drainage to obturator, internal iliac, and external iliac chains, which is why pelvic lymph node dissection accompanies radical cystectomy and why cross sectional imaging focuses on these basins.
