Description
Medial exposure of the brachial fascia frames the arm as a fibrous sleeve spanning from the axillary region to the elbow, investing the anterior and posterior compartments and sending intermuscular septa toward the humerus. Deep to this connective sheath lie the biceps brachii and brachialis anteriorly, with the triceps brachii forming the posterior mass, all oriented around the humeral shaft running superior to inferior. Along the medial aspect, the fascia thickens near the medial intermuscular septum, a partition that separates flexor and extensor groups and guides neurovascular structures toward the cubital fossa. Clean planes, clear relationships. That medial fascial plane matters in surgery and trauma because it dictates where you find, and how you protect, the basilic vein, medial cutaneous nerve of forearm, ulnar nerve (passing posterior to the medial epicondyle), and the brachial artery and median nerve as they course distally before bifurcation into the radial and ulnar arteries. Scar, hematoma, or compartment pressure within this closed sheath can distort surface landmarks and complicate approaches to humeral shaft fractures or distal biceps tendon repair, where dissection must respect the septa and the fascial continuity into the antebrachial fascia. This is the route where iatrogenic nerve injury happens.
