Posterior trunk anatomy is rendered with emphasis on the deep fascia investing the paraspinal musculature and blending laterally with the thoracolumbar fascia as it spans from the lower thoracic region toward the lumbar spine. Superficial back muscles remain identifiable, with trapezius positioned superiorly from the occipital region and nuchal ligament down toward the scapular spine, and latissimus dorsi lying more inferior and lateral, sweeping toward the humerus while crossing over the posterior thoracolumbar region. Along the midline, the fascial layers relate closely to the spinous processes, while laterally they thicken over the erector spinae compartment (iliocostalis, longissimus, and spinalis) and transition toward the abdominal wall and the posterior axillary fold. Planes matter here. For teaching and clinical anatomy, the deep fascia of the back is less about “wrapping” and more about force transmission and surgical planes: the posterior layer of the thoracolumbar fascia provides a strong aponeurotic interface between latissimus dorsi, gluteus maximus, and the paraspinal muscles, shaping load transfer during lifting and gait. That same fascial continuity becomes relevant during posterior approaches to the lumbar spine, when dissection follows the midline raphe and subperiosteal elevation exposes the spinous processes and laminae while trying to limit devascularization and postoperative paraspinal atrophy. Pain generators are often discussed here too, including thoracolumbar fascia strain in athletes and myofascial pain patterns that track along the paraspinal compartment.
